Private PPO and commercial plans are required to cover substance use disorder treatment at parity with medical care, so inpatient detox and residential rehab are typically covered once medical necessity is documented. Medical detox generally runs 3 to 7 days depending on how your body responds. Residential rehab most often runs 28 to 30 days, and insurers frequently authorize 45 to 90 days when clinically indicated.
